Technology and Facilities Access Request

This page is intended for individuals serving the students of the Westby Area School District in an official capacity. For facility rentals, please click here.

In keeping with the district's goal of a providing a safe and secure facility for students and staff, the forms below are designed to provide the appropriate access and resources to the appropriate individuals.


  • Please complete the Access Request web form below.
  • If you are requesting a badge, please have a photo taken in the District or School Office, or email a head shot taken on a neutral background in JPEG format to badges@westby-norse.org
  • The administrator/supervisor will sign off and the form will transmit to the IT department.
  • The IT Department will create all needed technology access and print your badge.
  • The IT Department will submit a copy of your form to the facilities office and your supervisor.
  • Keys, Badges, Passwords, and a copy of your form will be delivered to the office indicated on the form for you to pick up.
  • If you need keys, please submit a signed District Key Registration Form with your form - your key numbers will be added to the form & a copy given to you with your keys.
  • If you do not work in the district, all materials will be waiting for you at the building office indicated on the form unless you provide additional information on the form indicating where you would like to pick up the materials.
  • Please note that for non-district staff and temporary inpiduals, a $10 deposit is required for the badge, which will be returned to you when the badge is returned.

Forgotten User Credentials

  • Before submitting a Technology Support Request
    • Check the caps lock and number lock on your keyboard. 
    • Check network connectivity status.
    • Check spelling of username.
  • Submit a Technology Support Request Form indicating which service you are having trouble with accessing. 

Software Request

Please review the district's privacy policy before making a software request.
Once all criterion has been satisfied communicate with IT Services for the implementation of the software.

Staff Chromebook Troubleshooting and Repair

Submit a Technology Support Request indicating the issue.
To take a screen shot on a Chromebook press Ctrl switch window. (For non-Chrome OS keyboards, press Ctrl + F5.)
